We will happily bill your insurance company on your behalf. However, because of an incredible number of patients who have been unwilling to pay
their deductible, we are requiring that all patients pay the lesser of either their current outstanding deductible, or the cost of the office visit. This means that if there is no outstanding deductible for the current year, you will be expected to pay only your co-pay. For established patients, we may alternatively accept a signed credit card pre-authorization form, which would be executed only if you insurance company states that you are responsible for an outstanding balance and you have not submitted payment within 30 days of notification.
