Bronze Membership Plan
For a fee of $100 per month for 12 months, or a one-time annual fee of $1000, the member may see the physician (or his physician assistant) during regular business hours as often as he or she likes (with a $10 co-pay per visit). The membership includes only office visits, but does not include labs, procedures (such as EKG’s), medications or vaccines.
Silver Membership Plan
For a fee of $150 per month for 12 months, or a one-time annual fee of $1500, the member may see the physician (or his physician assistant) during regular business hours as often as he or she likes (with a $10 co-pay per visit). This level of membership also includes any procedures that the physician is able to perform in the office, such as EKG’s or repairs of any lacerations. It does not include labs, medications or vaccinations.
Gold Membership Plan
For a fee of $200 per month for 12 months, or a one-time annual fee of $2000, the member may see the physician (or his physician assistant) during regular business hours as often as he or she likes (with a $10 co-pay per visit). This level of membership includes any procedures that the physician is able to perform in the office, and also includes a 50% discount on all labs or medications that are normally available in the office, including injectable antibiotics (example, Rocephin). However, it does not include the cost of vaccines, which we could provide to you “at-cost.” (Please note, we provide most of our vaccines to all of our patients “at-cost” anyway, so you do not need to participate in any of our plans to get your vaccines “at-cost”).
Platinum Membership Plan
For a fee of $250 per month for 12 months, or a one-time annual fee of $2500, the member may see the physician (or the physician assistant) as often as her or she likes (with a $10 co-pay per visit). This level of membership includes any procedures that can be performed in the office, all labs, all medications normally available in the office, and all vaccines. It also includes full participation in our weight loss program, should you choose to take advantage of this.
All plans include 24/7 telephone access to the physician. Members will be provided the physician’s personal cell phone number, and his personal e-mail address. Additionally, for all memberships Silver and above, one free flu vaccination (when available) is included and one “base-line” EKG will be performed. Please Note: These plans are not intended to replace traditional insurance. They are simply offered as a supplement to existing insurance, or as an affordable alternative to those who cannot otherwise afford insurance. We realize that rising insurance premiums have put health insurance out of reach for many people; nevertheless, we still recommend that everyone have some insurance. At the very least, each of us should carry insurance for catastrophic illness or injury. This might be a high deductable plan that would provide coverage in case of a heart attack, a stroke, or an accident that might leave you hospitalized and/or disabled (as some examples). Often, by purchasing a high deductable plan, and paying for routine care out of pocket, patients can decrease their monthly premiums enough to make insurance somewhat affordable.
